1. Applicant must be permanent resident of the locality for which the fair price shop license is applied for.
2. Minimum Qualification is 10+2.
3. Not less than 21 years of age
4. The beneficiary is having Family ID/Parivar Pehchan Patra (PPP) number.

2. A resident of the locality for which the fair price shop license is applied for.
3. Basic Computer Knowledge Certificate
4. Minimum Qualification is 10+2.
5. However, in case of a Group like Self Help Group and Sakshar Mahila Smooh, the condition of 10+2 pass and age will not apply.
6. Undertaking regarding sound financial condition of the applicant.
7. Applicant must not be relative of existing Sarpanch/Panch in rural areas and MC/Deputy Mayor/Mayor/President/Vice-President in urban areas.
8. Undertaking regarding No Criminal case/EC Act case is pending against the applicant.
9. Undertaking regarding No applicant should be convicted in any criminal case.
10. Aadhar Card.
11. Undertaking regarding No family member has been allotted other FPS.
12. Panchayat Resolution in favour of the applicant, duly forwarded by the concerned BDPO.
13. Applicant should not be Government servant/Contractual basis or getting any Government pension.
14.  Proclamation Certificate of Chowkidar verified from village Panchayat/ MC.
15. Map of the Proposed Place for FPS.
16. Caste Certificate.
17. Rent Agreement (In case of Rented Property).

22. हालाँकि, स्वयं सहायता समूह और साक्षर महिला समूह जैसे समूह के मामले में, 10+2 उत्तीर्ण और आयु की शर्त लागू नहीं होगी।
23. आवेदक की मजबूत वित्तीय स्थिति के संबंध में वचन पत्र।
24. आवेदक ग्रामीण क्षेत्रों में मौजूदा सरपंच/पंच और शहरी क्षेत्रों में एमसी/डिप्टी मेयर/मेयर/अध्यक्ष/उपराष्ट्रपति का रिश्तेदार नहीं होना चाहिए।
25. आवेदक के विरुद्ध कोई आपराधिक मामला/ईसी अधिनियम मामला लंबित नहीं होने के संबंध में शपथ पत्र।
26. किसी भी आवेदक को किसी भी आपराधिक मामले में दोषी नहीं ठहराया जाना चाहिए।
27. आधार कार्ड।
28. परिवार के किसी भी सदस्य को अन्य एफपीएस आवंटित नहीं किए जाने के संबंध में शपथ पत्र।
29. आवेदक के पक्ष में पंचायत संकल्प, संबंधित बीडीपीओ द्वारा विधिवत अग्रेषित।
30. आवेदक सरकारी सेवक/संविदा आधारित या कोई सरकारी पेंशन प्राप्त करने वाला नहीं होना चाहिए।
31. ग्राम पंचायत/एमसी से सत्यापित चौकीदार का उद्घोषणा प्रमाण पत्र।
32. एफपीएस के लिए प्रस्तावित स्थान का मानचित्र।
33. जाति प्रमाण पत्र.
34. किराया समझौता (किराए की संपत्ति के मामले में)।
